CAD and post-myocardial patients require strong care coordination to assure effective secondary prevention and management. Most patients with CAD are managed by multiple providers across care continuum including both hospital and ambulatory settings. To assure effective care coordination, all team members managing these patients require access to the patient’s history, previous ECGs, imaging studies and treatment plan.
